We are seeking a caring, hard-working and reliable Nurse Practitioner (NP) or Physician's Assistant (PA).  We provide cutting edge treatments and are able to assist those patients that aren't getting better in a wound care facility or simply cannot travel to a facility due to travel difficulties.  On-the-job training and ongoing certifications will be a part of the on-boarding process for the right provider that wants to step out of a traditional hospital/clinical setting and become a part of our growing mobile wound care team.

Responsibilities Would Include:

*Provide advanced wound care treatments in home health, assisted living, nursing homes, long-term care and skilled nursing facilities.

*Conduct comprehensive wound assessments and develop personalized treatment plans

*Utilize evidence-based wound care techniques and implement advanced therapies

*Collaborate with physicians, home health agencies and facility staff to ensure seamless patient care

*Maintain accurate documentation an compliance with healthcare regulations

*Educate patients, caregivers and facility staff on wound prevention an best practices

*Adapt to a fast-paced, mobile healthcare environment while delivering high-quality, patient centered care

Qualifications and Requirements:

*Active Illinois Nurse Practitioner or Physician Assistant license

*Wound care experience a PLUS but not required
