Overview:

Sentara is hiring a Clinical Pharmacist at our Proprium Specialty Pharmacy in Chesapeake, VA.

Location: Proprium Pharmacy, Chesapeake, VA 23320

Status: Full Time (40 hrs/wk)

Shift: First

Hours: Rotates8:00am-5:00pm // 8:30am-5:30pm

The Specialty Clinical Pharmacist serves as a key member of the Ambulatory Clinical Support team, providing telephonic comprehensive medication management services to patients within Sentara’s Specialty Pharmacies. This role is responsible for ensuring safe, effective, and evidence-based use of specialty medications through patient counseling, therapy monitoring, and coordination with providers. By supporting adherence, optimizing therapeutic outcomes, and documenting interventions, the Clinical Pharmacist I helps advance quality care and patient-centered outcomes across complex disease states.

*This Pharmacist supports the call center team for Sentara's Specialty Pharmacies.

Education

  • Bachelor's Level Degree in Pharmacy (Required)

  • Doctorate Level Degree in Pharmacy (Preferred)

Certification/Licensure

  • Virginia Pharmacist License (Required)

  • Basic Life Support (BLS) (Required)

Experience

  • Minimum of 2 years experience in Clinical or Ambulatory Pharmacy (Preferred)*

  • Experience with MTM services (Preferred)

  • Collaborative care coordination, chronic complex disease patient counseling, and clinical assessment experience (Preferred)

  • Therigy EMR experience (Preferred)

*Completed Residency (PGY1 or PGY2) in lieu of Required experience
