Physical Therapist – Job Description

Position Summary:
The primary responsibility of this role is to help patients regain optimal physical function and support the achievement of individualized therapy and medical goals within a skilled nursing facility (SNF) setting. This includes developing and implementing treatment plans, providing patient education, and promoting long-term success through home exercise programs and activity modifications post-discharge.

Qualifications:

• Bachelor’s degree or higher in Physical Therapy from an accredited college or university

• Active, unrestricted state license to practice Physical Therapy

• Basic computer proficiency, including the ability to use therapy documentation systems and electronic health records
