Position Summary:

The Clinical Pharmacy Specialist – Population Health serves as a key clinical member of the SLMG Care Management Services team within the Population Health department. The Population Health Pharmacy Specialist leads pharmacist-driven disease management for high-risk and complex patients, with a specific focus on pharmacy-sensitive quality indicators including medication adherence, statin utilization, anticoagulation management, and chronic disease medication optimization. This role interfaces directly with Population Health leadership, SLMG physicians and care teams, and external payers as a clinical subject matter expert.

 

Education, Experience, & Licensing Requirements: 

Education: Doctor of Pharmacy (PharmD) from an ACPE-accredited College of Pharmacy required.

Experience: Minimum two (2) years of pharmacist experience in an ambulatory care, anticoagulation clinic, population health, or care management setting

Licensure: Active licensure as a Registered Pharmacist in the State of Missouri with medication therapy services (MTS) certification, in good standing, required.
