Position Summary: 

The Social Worker is responsible for helping patients and their families understand a particular illness, work through the emotions of a diagnosis, and provide counseling about the decisions that need to be made while working to mitigate social, financial and psychological problems related to their health condition. The Social Worker will work to match patient needs with appropriate resources and services to reduce risk of avoidable emergency room visits and rehospitalizations. The Care Coordination Department institutes measures to address high-risk complex patients, improve transitions of care, improve patient outcomes, and reduce hospital readmissions. This position is integral to St. Luke’s efforts to design, develop, and implement successful care delivery methodologies needed for patient care coordination across the healthcare continuum, alternative payment models and value-based care. The Social Worker will work in active partnership with physicians, office staff, administration and other health care providers to develop and implement a well-coordinated plan of care that promotes the right resources, at the right time and at the right level of care. The Care Management Social Worker is responsible for engaging and supporting patients that are in need of care management services. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke’s mission and values. 

 

Education, Experience, & Licensing Requirements:   

Education: Masters degree from accredited School of Social Work, Licensed Masters in Social work, Masters in counseling, or Provisional Licensed Professional Counselor. 

Experience:  One or more years of social work experience in a clinical setting, ambulatory or post-acute setting
