Mission:Ā  Center on Halsted advances community and secures the health and well-being of the LGBTQ people of Chicagoland.

Essential Functions:

As part of the Department of HIV & STD, IL Hotline at Center on Halsted (COH), the Health Educator will provide, in English and Spanish, health information and education, over the phone through the State of Illinois AIDS/HIV & STD Hotline; provide referrals, and links to care, accurately track all work and client information on paper and in secure databases. This service is over the phone and anonymous.

This position also requires HIV/STI screenings, Pre-Exposure Prophylaxis navigation services which include - conducting intakes, assessments, developing individual action plans, link to care, retention and re-engagement for people vulnerable to HIV acquisition. This service is in-person and not anonymous.Ā 

Experience working with the LGBTQ population in a sex positive environment preferred. This position requires extensive knowledge of HIV/AIDS and STDs.Ā 

This is an onsite, in-person position that requires ongoing availability to work days, evenings, nights, and weekends based on program needs. Availability for weekday and weekend shifts between the hours of 9:00 AM to 9:00 PM is required. This role may also require travel to outreach locations throughout the Chicagoland area.

Ā Job Description

Service Delivery Duties:

  1. ProvideH IV/PrEP/STDĀ counseling,Ā referrals, project case management,Ā and follow upĀ for walk-in,Ā scheduled,Ā online,Ā phone-based,Ā and outreachĀ clientsĀ in English and Spanish; provide such servicesĀ at COH and at various outreach locations; outreach on geosocial/dating appsĀ may beĀ required.
  1. Implement and oversee health education strategies, interventions, and programsthat address trauma, racial/ethnic inequities, and other social determinants of health
  1. Assess high-risk individuals; assist such individuals in decreasing their risk for contracting HIV and STDs; assess client needs for health education, including change readiness
  1. On-time arrival at Center on Halsted or in outreach settings as scheduled is imperative to ensure reliable client services at scheduled start time

Program Development and Collaborative Duties:Ā 

  1. In English and Spanish, translate scientific language and concepts into clear, simple and understandable information for the public and clients to maximize the potential for them to take reasonable action
  1. Helpidentify, develop, improve, and implement patient educationand/or advertisingmaterials and other tools that encourage healthy decisionsĀ 
  1. RemainĀ up-to-dateĀ on the latest HIV/AIDS and STD research through daily reading and studying of print and on-line materials, shared emails, monthly trainings, and attending seminars/conferences
  1. As needed and/or requested, assist the Director with special projects, presentations or training new Department staff

Ā Skills/Qualifications

  1. Results – proven track record of exceeding goals and bottom-line orientation; evidence of the ability to consistently make good decision through a combination of analysis, wisdom, experience, and judgement; the ability to balance the delivery of programs against the realities of a budget; ability to solve problems, work independently, manage programs; ability to be creatively resourceful is essential.Ā 
  1. Ability to obtain testing certification postĀ hire,Ā pre-certification is preferred.
  1. Experience working with individuals impacted by HIV/AIDS and STIs is required; prior experience working in a nonprofit environment is preferredĀ 
  1. Associate’s degree in social work or related field is preferred, or equivalent years of work/volunteer experience.
  1. Lesbian, gay, bisexual, transgender, and queer sensitivity; Knowledge and experience addressingLGBTQneeds.
  1. Excellent oral and written communications skills
  1. A high degree of integrity in dealing with confidential information.Follow and adhere to all HIPPA standards.
  1. Ability to use MSOffice programs, including MSWord, Excel and Outlook.
  1. Effective problem solving and ability to analyze and use data for decision-making.
  1. Effectively interact with people and develop positive relationships while being tactful, respectful, and direct in communication.
  1. Ability to work flexible hours including days (9am-5pm), nights (1pm-9pm shifts),weekends required.
  1. Data entry experience is required.
  1. Must be bilingual, specifically Spanish speaking(oral and written)Ā
