Role Overview
The Executive Director, Care Solutions is a high-visibility executive leader within Oak Street Health’s Population Health team. Reporting to the VP, Chief Population Health, this role is responsible for the end-to-end strategic oversight, financial modeling, negotiation, implementation, and ongoing performance of integrated care solutions. Key focus areas include evolving behavioral health models and scaling risk-bearing specialty care partnerships.
Key Responsibilities
Strategic Partnerships & Contract Negotiation
Serve as the primary subject matter expert on risk-bearing specialty care arrangements
Lead C-suite negotiations for complex, multi-million-dollar specialty care contracts that directly impact the care delivery P&L
Oversee the complete partnership lifecycle: opportunity scoping, due diligence, financial modeling, contracting, piloting, deployment, and continuous optimization
Partner regularly with CVS Ventures to share operational learnings and inform strategic investment decisions.
Behavioral Health & Care Model Innovation
Oversee the current Collaborative Care Model (CoCM) and lead the strategic evolution toward a fully integrated Primary Care Behavioral Health (PCBH) model.
Partner with Medical Leadership to ensure clinical fidelity, operational alignment, and seamless execution of evidence-based models.
Drive innovation in medical cost management through first-of-their-kind integrations across Primary, Behavioral, and Specialty Care.
Operational Execution & Governance
Direct daily partnership operations through a team of Lead Directors, connecting central strategy with clinical and field operations.
Partner with VP-level executives across Legal, Data, and Finance to conduct quarterly financial reconciliations and feed insights back into future partner negotiations.
Establish and monitor core KPIs—including patient engagement, program enrollment, and financial/clinical outcomes.
Design and establish secure access workflows for third-party partners to integrate into Oak Street Health clinical and operational systems.
Provide clear, actionable reporting to executive leadership regarding overall program operating conditions and financial performance.
Cross-Functional Leadership & Matrix Management
Direct, coach, and develop a team of director-level leaders, fostering a metric-driven and high-performance culture.
Collaborate with senior field leadership (Division Presidents, Executive Medical Directors, CMO, COO) to ensure smooth field implementation and clinical buy-in.
Drive cross-functional alignment among Population Health, Product, Data, Legal, Finance, and broader CVS Enterprise teams.
Capabilities We're Looking For:
Population Health & Risk Knowledge: Deep understanding of Risk-Bearing Entities, value-based delivery models, and population health management strategies.
Specialty & Behavioral Health: Comprehensive knowledge of virtual specialty care delivery models, behavioral health integrations, and clinical best practices.
Payer & Regulatory Knowledge: Solid familiarity with Medicare regulations, reimbursement, and Medicare Advantage frameworks.
Partnership Management: Proven track record of sourcing, evaluating, structuring, and negotiating complex agreements with external partners and vendors.
Data-Driven Execution: Strong analytical background with the ability to leverage data to optimize clinical workflows, care outcomes, and financial performance.
Background Preference: Prior management consulting experience in healthcare is highly valued.
Executive Communication: Exceptional written, verbal, and interpersonal communication skills, with a proven ability to distill complex clinical and financial data for C-suite executives and matrixed operational teams.
Adaptability & Drive: High-agility self-starter who thrives in fast-paced, growth-stage, or high-ambiguity environments.
Relationship Building: Demonstrated ability to establish credibility and forge strong, collaborative partnerships across both clinical and business leadership.
Problem-Solving & Execution: Exceptional organizational skills with a solution-oriented mindset and a track record of executing multi-stream initiatives under tight deadlines.
Required Qualifications
10+ years of experience in Value-Based Care, Population Health, or Healthcare Delivery organizations.
Proven track record negotiating complex, multi-million-dollar risk-bearing contracts with third-party providers or vendors.
Deep domain expertise in Behavioral Health integration (CoCM, PCBH) and high-spend Specialty Care risk arrangements.
Strong financial acumen with direct exposure to P&L management, financial reconciliations, and complex financial modeling.
Demonstrated ability to navigate matrixed healthcare organizations and drive engagement among C-suite executives and clinical leaders.
Education:
Bachelor's Degree or equivalent combination of education and relevant work experience (required)
Master's Degree (preferred)
Pay Range
The typical pay range for this role is:
$131,500.00 - $303,195.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.Â
