Job DetailsJob Location: Chicago, IL 60642Salary Range: $60,000.00 - $63,000.00 Salary Erie Neighborhood House, a community-building agency empowering Chicago’s low-income and immigrant families, and dedicated to promoting a just and inclusive society is seeking a Health Equity Program Manager.  The Health Equity Manager’s basic function is to manage the Health Equity Program and will work closely with the Health Educator and Community Health Worker to coordinate activities between the programs and department staff within the agency and with external partners. The mission for the program to promote a culture of health and encourage Erie’s staff and participants to become leaders in the movement for community wellness and equity. The Health Equity Manager is responsible for the strategic implementation and oversight of community-based health equity initiatives that address social determinants of health and improve access to care. This leadership role supervises the Health Educator and Community Health Worker and manages programs funded by grants such as ILCHF, WSHEC, health and nutrition initiatives, and the community health worker and health promoter projects. The Health Equity Manager ensures that programs are implemented effectively, comply with funder requirements, and align with Erie House’s mission to promote health and wellness in diverse communities. POSITION SUMMARY Program Leadership & Oversight Supervise and support the Health Educator and Community Health Worker, including performance reviews, coaching, and day-to-day guidance. Oversee the implementation of health equity programs to meet strategic goals and community needs. Ensure coordination and integration of services across sites and programming. Grant & Compliance Management Manage the activities and compliance of all health-related grants (i.e.Wellness West (WSHEC), nutrition, CHW, health promoters). Collaborate with development and finance teams on grant applications, renewals, budgets, and reports. Ensure timely, accurate reporting and documentation in line with funder requirements. Data, Evaluation & Reporting Monitor and analyze program data to assess progress toward health equity outcomes. Collaborate with the Data and Outcomes Administrator to maintain accurate data systems and reporting tools. Generate reports for internal stakeholders and funders. Community Engagement & Capacity Building Engage with community members and stakeholders to assess needs and build partnerships. Support and oversee outreach, workshops, and health education activities. Identify trends and emerging needs in community health to inform programming. Administrative & Operational Management Facilitate team meetings, scheduling, and coordination across departments. Address challenges related to program implementation and staffing. Contribute to strategic planning and operational improvements. Other: Assist with other duties as assigned. Qualifications Bachelor’s degree in Public Health, Social Work, Health Administration, or equivalent of 2 years of experience in non - profit management  At least 2 years of experience in health programming, community health, grant coordination, non-profit management Experience supervising or leading teams preferred. Strong organizational, communication, and interpersonal skills. Proficient in program data collection, analysis, and reporting. Bilingual in English and Spanish preferred. Commitment to social justice and familiarity with immigrant and underserved communities. Flexibility to work evenings/weekends as needed and travel locally.
