COMPENSATION: The hiring range for this position is $70,000–$90,000, with the opportunity to earn additional compensation through bonuses; candidates may also choose between a base salary structure or a variable compensation plan (commission), which may provide higher earning potential

Position Summary:
Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.Â
Key Responsibilities:
- Conduct comprehensive psychosocial assessments for residents upon admission and as neededÂ
- Develop, implement, and monitor individualized treatment plansÂ
- Provide individual, group, and/or family therapy sessionsÂ
- Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disabilityÂ
- Support residents coping with chronic illness, pain, loss of independence, and end-of-life concernsÂ
- Collaborate with physicians, nursing staff, social workers, and rehabilitation teams to coordinate careÂ
- Participate in care plan meetings and interdisciplinary team discussionsÂ
- Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulationsÂ
- Provide crisis intervention and behavioral management strategies when neededÂ
- Educate staff and families on mental health conditions and effective communication strategiesÂ
- Ensure compliance with HIPAA and ethical standards of practiceÂ
QualificationsÂ
- Master’s degree in Psychology, Counseling, Social Work, or related fieldÂ
- Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)Â
- 2+Â years of clinical experience preferred, ideally in geriatric, long-term care, or medical settingsÂ
- Strong clinical assessment, diagnostic, and treatment planning skillsÂ
- Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)Â
- Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)Â
- Ability to work collaboratively within an interdisciplinary teamÂ
- Excellent communication and interpersonal skills with residents, families, and staffÂ
- Strong organizational, time management, and documentation skillsÂ
- Familiarity with electronic health records (EHR) systems preferredÂ
