Day Shift Needs:

• Full-time/part-time QMAP.

Evening Shift Needs:

• Part-time QMAP

Night Shift Needs:

• Full-time QMAP.

Additional Needs:

• PRN PCP/QMAP staff.​

Other Information:

• New hires required to work every other weekend!

• Flexible scheduling.

• Starting pay: $17-22/hr. depending on experience.

About the Role:

As a Personal Care Provider/QMAP at Lakeview Commons, you will:

• Help residents build and maintain meaningful relationships by facilitating group activities.

• Allow residents to maintain their dignity by assisting with activities of daily living such as bathing, dressing, meals, and maintaining a clean-living environment.

• Accurately document and track care plans including restorative care under the direction of various therapists.

What We're Looking For:

Experience:

• Interest in geriatrics required; previous experience preferred, but not required.

• Must be willing to attend QMAP classes with the follow-up expectations of administering medication.

Skills:

• Ability to read, write, speak, and follow oral instructions in English.

• Attention to detail in all aspects of care provision.

• Capability to work both independently and in a team environment.

Attributes:

• Compassionate and empathetic nature with a desire to care for others.

• Reliable with a strong work ethic.

• A team player who fosters and promotes a positive and supportive work environment.

Our Total Rewards package includes a salary range of between $17 - $22/hr. DOE
