As a Sr. Manager, you will play a critical role in supporting our Oak Street Health patients, providers and business by supporting the successful execution, adoption, and operationalization of care management operations and new care management initiatives across Oak Street Health centers.
This role serves as a key bridge between centralized care management strategy and field operations, partnering closely with center and regional leaders to drive understanding, alignment, and adoption of evolving care management workflows, processes, and team structures.
The ideal candidate is a highly collaborative operational leader with deep knowledge of interdisciplinary care management models and strong experience navigating complex operational and organizational change in a fast-paced healthcare environment.
This role is focused on field enablement, operational execution, continuous improvement, and cross-functional partnership—not direct clinical precepting or direct people management of care management staff.
Key Responsibilities:
Field Enablement & Adoption
Support implementation and operationalization of care management workflows, programs, and strategic initiatives across Oak Street Health centers
Partner with center, market, and regional leaders to improve understanding and adoption of evolving care management workflows and team structures
Reinforce role clarity and interdisciplinary collaboration across care management functions, including RN-CM, MSW, CHW, TCM, BHS, and center operations
Identify adoption barriers, operational pain points, and workflow inconsistencies in the field
Operational Partnership
Serve as a trusted operational partner to field and center leadership teams
Facilitate regular touchpoints with field stakeholders to gather feedback, identify trends, and surface emerging operational needs
Support centers through periods of operational change and implementation of new initiatives
Translate strategic initiatives into practical operational workflows and field-ready execution plans
Cross-Functional Collaboration
Collaborate across centralized operational, clinical, training, and implementation teams to develop scalable solutions to field challenges
Act as a feedback loop between field teams and centralized stakeholders to ensure continuous improvement
Partner with internal teams to refine workflows, tools, communication strategies, and support resources based on field experience
Continuous Improvement & Change Management
Monitor field adoption and operational effectiveness of care management workflows and initiatives
Identify opportunities to improve consistency, efficiency, and execution across markets
Support development of best practices and operational standards
Contribute to change management planning and execution for future care management initiatives
Required Skills & Competencies
Strong understanding of interdisciplinary care management operations and workflows
Ability to influence and collaborate effectively across multiple levels of leadership
Strong operational problem-solving and process improvement capabilities
Exceptional communication and relationship-building skills
Experience leading or supporting large-scale operational or organizational change initiatives
Ability to navigate ambiguity and operate effectively in evolving environments
Strong organizational skills with the ability to manage multiple priorities simultaneously
Demonstrated ability to synthesize field feedback into actionable operational solutions
High level of adaptability, accountability, and execution focus
Preferred Experience
Experience in healthcare operations, care management, population health, or value-based care environments
Familiarity with interdisciplinary care team structures and workflows
Experience working closely with field operations and center leadership teams
Experience supporting multi-site operational initiatives or implementations
Experience supporting rollout and adoption of new operational workflows or care management programs preferred
Education & Qualifications
Bachelor’s degree required
Master’s degree in Healthcare Administration, Public Health, Business Administration, Nursing, Social Work, or related field preferred
5+ years of experience in healthcare operations, care management operations, implementation, or field support roles
Willingness to travel regularly to markets and centers as needed (~25%)

Pay Range
The typical pay range for this role is:
$82,940.00 - $182,549.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.Â
