Sage Group HealthCare Staffing is seeking a Hospice Registered Nurse for a 13-week travel/contract position in Chillicothe, OH. We are accepting both local and traveler candidates.

Pay Rate (Days): Range $2360 to $2440 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home Health, Hospice

Shifts available: Evenings

Schedule: 40 hours/week guaranteed, Mon-Thu, 3.30 pm to 2.00 am (on call support with field visits as needed)

Position Requirements:

• Registered Nurse: 1-2 years - Required
  
• Recent/Current Hospice Nursing experience in a home health setting: 1 year within the last 3 years – Required
  
• Home Care Home Base (HCHB) charting experience – Required 
  
• Must have a valid Driver’s License and Personal Auto Insurance
  
• Covid Vaccination (2 doses) Proof - Required, Declinations not accepted, Exemptions may be considered on a case to case basis
  
• Must be able to provide Immunization records or titers for MMRV and HepB vaccination upon offer prior to start
  

Education, License and Certification Requirements:

• ASN or an equivalent – Required
  
• BSN or an equivalent – Highly preferred
  
• RN License for State of Ohio or Compact - Required
  
• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide On-call support and field visits as needed
  
• Provide and coordinate care for assigned hospice patients in comfort of their homes.
  
• Develop and implement nursing care plans for assigned patient case load
  
• Support patient comfort and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
  
• Empower patients and families to reach healthcare goals by educating them on disease management.
  
• Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  
• Notify physicians of significant changes in status.
  
• Document and complete charting for all patient visits within 24 hours.
  
• Provide bereavement support to patient’s families as needed.
