This is your opportunity to make a lasting impact, not just on a team, but on the lives of those we serve.   If you're a compassionate Registered Nurse with strong leadership skills and a passion for supporting both patients and clinicians, we’d love to meet you.  QualificationsRequirements & Responsibilities:  Graduate of an accredited nursing program (Associates, Bachelors, or Masters)  Current Registered Nurse (RN) license in the state of practice  2+ years of hospice or home health clinical experience required  2+ years of supervisory or management experience required  Oversee coordination of patient care and assign appropriate staff to each case  Support clinicians with real-time guidance, mentoring, and clinical problem-solving  Conduct case reviews, home visits, and clinical audits to ensure high-quality care  Manage hiring, training, supervision, and evaluation of clinical staff  Assist in ongoing staff education, orientation, and performance improvement efforts  Must have reliable transportation, valid driver’s license, and proof of auto insurance  Benefits & Schedule:  Schedule: Monday through Friday, full-time position with on-call responsibilities  Health, dental, vision, and life insurance  Company-paid short/long-term disability and basic life insurance  401(k) with up to 4% company match  Tuition reimbursement and professional development opportunities  Paid time off and paid holidays
