The Behavioral Health Professional is an active member of the primary care treatment team. They are responsible for providing consultation to the primary care team and developing therapeutic relationships that include addressing the complex bio-psychosocial needs of the patient.  The scope of practice includes but is not limited to conducting behavioral health screenings, comprehensive assessment, diagnosis, treatment planning, advocacy, and resource identification and linkage. Develop therapeutic relationships with patients to address their bio-psychosocial needs. Conduct behavioral health screenings and comprehensive assessments. Formulate diagnoses and develop individualized treatment plans. Provide evidence-based therapeutic interventions. Engage in crisis intervention and client advocacy. Perform case management and ensure continuity of care. Identify and link patients to appropriate community resources. Maintain accurate and timely documentation of patient interactions and treatment plans. Participate in interdisciplinary team meetings and collaborate with other healthcare providers. Stay current with state and federal regulations related to healthcare and behavioral health services. Promote patient involvement and motivation to change behavior. QualificationsEducation: Master’s Degree in Social Work, Mental Health Counseling, or another human services master’s degree. (Required) Experience: One to two years of experience in behavioral health. (Required) Broad background with diverse patient populations. Licenses and Certification: Relevant state licensure or certification in social work, mental health counseling, or a related field. Professional Skills and Abilities: Knowledge of professional human services principles, methodology, and ethics. Understanding of human psychosocial development within the family, community, and culture. Familiarity with brief therapy theories and techniques. Understanding of techniques for facilitating client motivation to change behavior. Knowledge of health and social issues impacting diverse clients. Knowledge of state and federal rules, regulations, and requirements related to healthcare. Skill in providing evidence-based practices. Skill in crisis intervention and client advocacy. Skill in case management. Ability to interact effectively as a member of an interdisciplinary healthcare team. Ability to identify and utilize community resources. Ability to communicate appropriately with a diverse patient population. Physical Requirements: Ability to sit, stand, and walk for extended periods. Manual dexterity to perform clinical tasks. Visual and auditory ability to perform job functions. Ability to handle stressful situations and manage crisis intervention. Capacity to travel to various locations as needed for patient care and collaboration.
