The Case Manager for Adult Mental Health Services, provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.

Salary Range: $45,979.65-$52,876.60 - Depending on Experience

 Education/Experience Required:Â
- Bachelor’s degree in education, social work, psychology or human services related field.
- Registration as a QMHP or trainee is preferred.
