JTC Home Health Inc. is a growing, locally-owned home health company dedicated to providing high-quality, patient-centered care throughout the San Diego area. We are looking for a compassionate and skilled Speech-Language Pathologist (SLP) to join our team on a per diem, fee-for-service basis.
This role offers exceptional flexibility and competitive compensation, allowing you to manage your own schedule while providing one-on-one therapy in the comfort of our patients' homes.
Responsibilities
- Conduct comprehensive initial and ongoing speech-language pathology evaluations and assessments in the patient’s home environment.
- Develop, implement, and document individualized treatment plans (POC) for patients with speech, language, cognitive, and swallowing disorders (dysphagia).
- Provide patient/caregiver education and training on home programs and compensatory strategies.
- Coordinate with physicians, nurses, and other members of the interdisciplinary healthcare team.
- Complete timely and accurate OASIS and documentation requirements using an Electronic Medical Record (EMR) system.
- Maintain open and professional communication with the JTC Home Health office team.
Qualifications
- Master's degree in Speech-Language Pathology from an accredited program.
- Current and unencumbered California SLP license.
- Certificate of Clinical Competence (CCC-SLP) from ASHA required.
- Minimum of one (1) year of experience as an SLP, with at least six (6) months in a certified Home Health agency preferred.
- Knowledge of Medicare/Medicaid regulations and documentation requirements (OASIS experience preferred).
- Must have a reliable car, valid driver's license, and current auto insurance.
- Ability to travel throughout San Diego and surrounding counties.
