At Talbert House, we’re on a mission to provide compassionate, client-centered care—and we’re looking for a dynamic Housing Case Manager to help us lead the way.  As a Housing Case Manager, you’ll be a vital part of our team—working directly with clients to support them in achieving stability, autonomy, and overall wellness. From assessment to care coordination, your work will touch every part of a client’s journey, making an impact that truly matters.

What You’ll Do:

  • Coordinate and deliver high-quality, timely, and equitable care in collaboration with offsite support

  • Maintain a caseload and stay connected with clients through ongoing contact and progress monitoring

  • Create individualized service plans and support clients through advocacy, crisis intervention, and referrals

  • Provide assistance with housing, budgeting, transportation, medication monitoring, and more

  • Facilitate group sessions or educational workshops as needed

  • Document clinical activity clearly and on time to meet all compliance standards

  • Participate in treatment planning and contribute to decision-making on client care

  • Help individuals move toward independence through supportive counseling and resource linkage

What You Bring:

  • A Bachelor’s Degree in Behavioral Science or related field, or equivalent experience (as permitted by regulations)

  • Valid driver’s license and auto insurance

  • Strong interpersonal and communication skills—you connect with people from all walks of life

  • The ability to stay organized, manage multiple responsibilities, and solve problems with confidence

  • Comfort using technology and electronic documentation systems (Microsoft Office proficiency preferred)


Essential Functions/Physical Demands:

  • Positional: Driving in accordance with job duties assigned. Occasional standing and walking. Occasional to frequent sitting.
  • Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching and handling.
  • Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
