About the Role

As a Physical Therapist at Concept Rehab, you will provide evidence-based evaluation and treatment to patients recovering from illness, injury, or surgery in a post-acute rehabilitation setting. You’ll collaborate closely with interdisciplinary teams, patients, and families to help individuals regain mobility, strength, and independence.

This role is ideal for PTs who value clinical quality, teamwork, and professional development.

Requirements

Key Responsibilities

• Evaluate patients within 24–48 hours of physician orders
• Develop and implement individualized treatment plans
• Provide skilled, patient-centered therapy interventions
• Supervise and collaborate with Physical Therapy Assistants
• Communicate effectively with physicians, nursing, families, and care teams
• Participate in interdisciplinary and care planning meetings
• Document evaluations, treatments, and progress accurately and timely
• Support discharge planning and caregiver education
• Participate in staff education and in-service training as appropriate

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy
• Active Ohio Physical Therapist license
• Strong communication and collaboration skills
