Mobile Wound Care Specialist – Physical Therapist Assistant (PTA) Position Summary The Mobile Wound Care Specialist – PTA provides specialized wound care support across assigned skilled nursing, long-term care, and assisted living facilities. This position works collaboratively with physical therapists, occupational therapists, nurses, physicians, wound care providers, and facility leadership to support the assessment, treatment, prevention, and management of wounds within the PTA's scope of practice and applicable state and federal regulations. The Mobile Wound Care Specialist is responsible for providing consistent, high-quality wound care services across multiple locations while promoting evidence-based practices, positive clinical outcomes, regulatory compliance, and interdisciplinary collaboration. Essential Responsibilities Wound Care & Clinical Services Provide wound care interventions within the PTA's legal and professional scope of practice and under the direction and supervision of a licensed Physical Therapist. Assist with implementation of individualized wound care treatment plans established by the supervising therapist and/or other qualified healthcare professionals. Perform wound-related treatments and interventions for appropriate residents based on orders, the plan of care, facility policy, and scope of practice. Monitor wound characteristics including size, drainage, tissue appearance, odor, periwound condition, and changes in skin integrity. Accurately document wound status, interventions provided, resident response, and progress toward established goals. Utilize approved wound care modalities and technologies as trained, authorized, and permitted by applicable regulations. Identify changes in wound condition and promptly communicate concerns or significant changes to the supervising therapist, nursing staff, physician, or other appropriate provider. Assist with pressure injury prevention and skin integrity programs. Promote positioning, mobility, offloading, pressure redistribution, and other therapeutic strategies that support wound healing and prevention. Mobile/Regional Responsibilities Travel between assigned facilities to provide wound care services and clinical support. Maintain an organized schedule and prioritize resident needs based on clinical urgency and facility requirements. Establish effective working relationships with therapy and nursing teams at each assigned location. Provide support to facilities experiencing increased wound care needs, staffing challenges, or complex wound populations. Assist with implementation and standardization of wound care practices across assigned facilities. Communicate effectively with facility leadership and regional therapy leadership regarding wound care needs, trends, barriers, and opportunities for improvement. Interdisciplinary Collaboration Collaborate with nursing, therapy, physicians, wound care providers, dietitians, dietitians, and other members of the interdisciplinary team. Participate in wound rounds, care conferences, and other interdisciplinary meetings as appropriate. Educate residents, families, caregivers, and staff regarding skin integrity, pressure injury prevention, positioning, mobility, and wound care strategies within the PTA's scope. Promote continuity of care by ensuring appropriate communication during transitions between therapy, nursing, medical providers, and other disciplines. Documentation & Compliance Complete all required documentation accurately, timely, and in accordance with company policy, payer requirements, facility procedures, and applicable regulations. Maintain appropriate records related to wound assessments, treatments, resident response, and communication with the interdisciplinary team. Follow all applicable federal, state, professional, facility, and organizational requirements related to wound care and PTA practice. Maintain confidentiality and comply with HIPAA and organizational privacy standards. Demonstrate appropriate understanding of Medicare, Medicaid, managed care, and other applicable payer requirements. Maintain competency in all assigned wound care modalities and procedures. Quality Improvement & Education Identify trends in wound development, healing, recurrence, and prevention and communicate findings to appropriate leadership. Assist facilities with quality improvement initiatives related to wounds, pressure injuries, falls, mobility, positioning, and skin integrity. Provide education and mentorship to therapy and facility staff regarding appropriate wound prevention and treatment strategies within the PTA's scope. Participate in ongoing education to maintain current knowledge of wound care practices, evidence-based interventions, and regulatory requirements. Support company initiatives designed to improve clinical outcomes, resident experience, and quality of care.   Qualifications  Education Must possess an Associate degree from an accredited program conferring the title of Physical Therapist Assistant.   Physical and Sensory Requirements  (With or Without a Reasonable Accommodation) Must be able to move intermittently throughout the workday. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met. Must meet the general health requirements set forth by the policies of this Facility, which may include a medical and physical examination. Based on the Occupational Safety and Health Administration's Guidelines for Nursing Homes Ergonomics for the Prevention of Musculoskeletal Disorders and the American Conference Governmental Industrial Hygienists' Threshold Limit Values for Lifting the Facility has identified that this job may require the lifting of residents, equipment, or other objects.  Accordingly, this job may require a minimum of 5 pounds and a maximum of 45 pounds lifting, periodically and or as needed. Specific Requirements   Required Graduate of an accredited Physical Therapist Assistant program. Current PTA license in good standing in the state of OH. Current CPR/BLS certification as required by the organization. Previous clinical experience in a skilled nursing, long-term care, rehabilitation, outpatient, or similar healthcare setting. Demonstrated knowledge of wound prevention, skin integrity, positioning, mobility, and therapeutic interventions. Ability and willingness to travel between multiple assigned facilities. Valid driver's license and reliable transportation. Preferred Wound care certification or specialized wound care training. Experience treating complex wounds in a skilled nursing or long-term care population. Experience with wound care modalities and advanced wound management technologies. Experience providing staff education or clinical training. Experience working in a mobile, regional, or multi-facility clinical role. Commitment to regulatory compliance and high-quality patient care. Physical Requirements Ability to frequently stand, walk, bend, stoop, kneel, reach, and move throughout a healthcare facility. Ability to safely assist with positioning, transfers, mobility, and other therapeutic activities as required. Ability to lift and/or assist with lifting according to organizational and facility safety requirements. Ability to use computers, electronic medical records, and other technology required for documentation and communication. Ability to travel between assigned facilities as required.
