- Monitor for rejection, infection, and complications via labs, imaging, and clinical visits.
- Manage immunosuppression therapy in coordination with providers and pharmacy.
- Educate patients and caregivers on medication adherence, infection prevention, and lifestyle changes.
- Coordinate specialty consults, urgent evaluations, and transitions of care.
- Maintain regulatory documentation for adverse events, graft loss, and outcomes.
- Participate in protocol development, quality projects, and research initiatives.
