 Pre-Transplant Coordinator
  • Initiate, coordinate, and track pre-transplant evaluations and required testing.
  • Prepare evaluation charts, review histories, and synthesize findings for committee presentation.
  • Provide education and obtain informed consent for evaluation and listing.
  • Collaborate with referring providers, cardiology, and multidisciplinary teams.
  • Ensure accuracy and compliance of UNOS listing data, closure letters, and notifications.
  • Manage evaluation communication channels (phones, faxes, imaging transfers).
  • Support regulatory adherence and process improvement in evaluation workflows.
