 Pre-Transplant Coordinator
- Initiate, coordinate, and track pre-transplant evaluations and required testing.
- Prepare evaluation charts, review histories, and synthesize findings for committee presentation.
- Provide education and obtain informed consent for evaluation and listing.
- Collaborate with referring providers, cardiology, and multidisciplinary teams.
- Ensure accuracy and compliance of UNOS listing data, closure letters, and notifications.
- Manage evaluation communication channels (phones, faxes, imaging transfers).
- Support regulatory adherence and process improvement in evaluation workflows.
