Registered Nurse (RN) Clinical Field Staff Liaison - Hybrid work opportunity
Schedule: Monday - Friday, 8:15 AM - 5:00 PM + holiday rotation

NOTE - After 3+ months of training, will be able to work 3 days from home and 2 days in office!!
SUMMARYÂ
The RN Clinical Field Staff Liaison (CFSL) provides support and acts as a resource to clinicians in the field. The CFSL assists in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management. The CFSL is responsible for all practices and duties within the scope of practice as outlined by the state.Â

KEY RESPONSIBILITIESÂ
Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concernsÂ
Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to managerÂ
Work in the field with staff and patients as neededÂ
Maintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assistedlivings and familiesÂ
Assist with monthly Case Manager, Nurse and Therapy meetingsÂ
Ensure Start of Care complianceÂ
Ensure quality and safe delivery of home health servicesÂ
Monitor Home Health Aide supervisory visitsÂ
Focus on Outcome Monitors as assignedÂ
Implement quality improvement programs in the fieldÂ
Completes field visits as neededÂ
Participates in managing cost of good working with monitoring supply costs and visits per episodeÂ
Knowledgeable of CFSS workflowÂ
Review relevant reportsÂ
Reviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesÂ
Maintains active RN license in state of employmentÂ
Participates in on-call, evening/weekend shifts and provides patient care, per agency needsÂ
Participates in in-service program developmentÂ
Maintains a professional appearance as a representative of the companyÂ
Understands, implements and practices policies and procedures for agency operationÂ
Other duties as assignedÂ

QUALIFICATIONSÂ
Registered Nurse with current license in the state of employment. Compact licensure, for states that participate, is required to be obtained within first 30 days of employment for additional support in all ASHN affiliated agencies.Â
Three years of experience as an RN in an acute care settingÂ
Minimum one-year home health experience is preferredÂ
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workersÂ
Ability to remain calm, have patienceand be accommodating. Compassionate and caring while working with patientsÂ
Knowledgeable on nursing best practicesÂ
Ability to make appropriate nursing judgmentsÂ
Ability to identify a situation and handle it with the best possible solutionÂ
Detail-oriented and observantÂ
Disciplined style of work ethic with the ability to prioritize and be timelyÂ
Ability to follow directions and work as a team memberÂ
Knowledge of MS Office applications including Word, Excel, PowerPoint and OutlookÂ
Valid driver’s license and auto insurance with your name as a listed driverÂ
EDUCATION AND CREDENTIALSÂ
