The role:

As a key frontline team member at Better Life Partners, you will serve as a primary point of contact for members, helping them understand, coordinate, and access the care and support they need. You’ll embody our core values by delivering compassionate, organized, and responsive support to ensure every member feels seen, heard, and cared for.

 

This is a Remote position. Unless otherwise agreed upon, all candidates must reside in-market. Our team provides services Monday through Saturday, 8:30 a.m. to 8:30 p.m. Eastern Standard Time. Shifts are 8 hours. Team members must be able to work at least three evening shifts per week. The work schedule will be Monday, Tuesday, Thursday 12:00pm-8:00pm, Wednesday and Friday 9:00am-5:00pm.

What you will do:
  • Demonstrating and communicating the core values of Better Life Partners in every interaction
  • Conducting initial assessments to identify members’ social, behavioral, and medical needs
  • Creating and executing personalized care navigation plans, tailored to help members access critical resources and services
  • Guiding members through complex healthcare systems, including scheduling appointments, referrals, follow-ups, and care coordination
  • Reviewing relevant member information prior to each session and sharing it with Counselors and Medical Providers to ensure coordinated care
  • Greet members virtually and assist them with accessing and navigating the virtual meeting platform
  • Providing technical support and troubleshooting to members experiencing issues with the virtual care platform.
  • Handling new and returning member intakes, including onboarding and appointment scheduling
  • Meeting members’ social needs, such as housing, food, transportation, and other critical resources, by identifying barriers to care and connecting them with appropriate services
  • Communicating proactively with internal teams (Counselors, Providers, Leadership, and the Member Services Team) to ensure alignment on member needs and status
  • Documenting all member interactions accurately and in a timely manner in the electronic health record (EHR) and other tracking systems
  • Providing education and resources to support member' understanding of their care plans and improve health literacy.
  • Supporting special operational projects and contributing flexibly to organizational needs with an "all hands on deck" attitude


You are a good fit if you have:
  • A High School Diploma or equivalent (required)
  • Willingness and eagerness to learn, be flexible, and assist teammates when needed
  • Excellent time management and organizational skills
  • Strong work ethic and reliability
  • Ability to multitask in a fast-paced, team-oriented environment
  • Reliable internet access and strong computer skills
  • Certified Recovery Support Worker (CRSW) credential (preferred)


It will also be great if you have:
  • A Bachelor's Degree or equivalent work experience
  • Passion for working in the healthcare field or substance use recovery
  • 1+ year(s) experience in a Medication-Assisted Treatment (MAT) program
  • 1+ year(s) experience in case management or care navigation


Work location requirements:
  • BLP operates in hybrid and remote work environments, which allows us to better meet our members and partners where they’re located. Candidates applying for this role must be willing and able to travel locally within the communities we serve and/or travel to onsite meetings as expected in coordination with their department and business needs.


When working from home, the requirements include:
  • Must have reliable internet service with a fast upload/download ability
  • Ability to ensure that any protected health or proprietary data/information is not visible (or audible)  to others in any work location
  • Must have a quiet space to speak to members, team members, or external partners with minimal background noise and distractions

$20.22 - $25.16 an hour
The actual compensation offered within the range, along with title and level, will be dependent on a variety of factors including, but not limited to, relevant experience, qualifications, skills, level offered, business requirements, and performance expectations.
