We offer two unique care settings, giving you the opportunity to choose the path that fits your passion and grow your career along the way:   Sacred Heart Home Assisted Living   In this setting, LPNs take on a leadership-focused, care coordination role where relationships and oversight are key, while staying actively involved in the day-to-day care of residents.   What you’ll do: Coordinate resident care and monitor overall wellness Oversee and participate in medication administration and treatments Respond to changes in condition, incidents, and daily needs on the floor Serve as a coach and supervisor for HHAs, PCAs, and Medication Technicians Communicate with families and the interdisciplinary team to ensure seamless care and follow-through Support a home-like environment where independence and dignity are prioritized   Why you’ll love it: A balanced, team-oriented and supportive environment where relationships matter, and where you can stay engaged in daily care while leading and supporting those around you.   Brothers of Mercy Nursing & Rehabilitation Center Prefer a more hands-on clinical role?
