As a Social Worker, you’ll be a key member of our interdisciplinary care team, guiding patients and families through the discharge process and ensuring a smooth transition from our sub-acute rehab program to the next stage of care. Qualifications Bachelor’s Degree in Social Work  Experience in long-term care or sub-acute/rehab setting preferred Knowledge of discharge planning, community resources & MDS 3.0 Strong communication, organizational, and interpersonal skills Compassion and commitment to serving the elderly population
