Job DetailsJob Location: Powell Outreach/ Foothills Active Day Center - Clay City, KY 40312Salary Range: $20.00 - $22.00 HourlySummary: Under the general supervision of the Clinical Director/ Clinic RN/BSN Nurse Manager, applies nursing skills in the care and treatment of patients while also conducting outreach to and advocacy for the homeless population. The RN/LPN will be responsible for providing quality care of patients in a safe, effective, efficient manner and to act as a liaison between clients and their families and health, mental health, and social service providers. Also, coordination of medications ordered through Drug Assistance Program. In addition, the RN/LPN assists the Foothills Active Day Center Director with management responsibilities, including monitoring the health needs of participants as needed.     Essential Duties:    Assists in triaging walk-in and call-in patients.  Coordinates patient flow through clinic.  Provides direct patient care and patient education services in the clinic.  Interviews patients; documents medical history; examines patients and makes assessments and/or physical diagnosis of condition of such patient, counsels patient with regard to treatment and prevention.   Interprets and reports observations, in keeping with sound LPN practices.   Assess patient health status and notify providers of patients who require urgent attention.  Administers medications and treatments via established protocols as authorized by licensed physicians or nurse practitioners.   Answers the telephone and provide information about the clinic, routes clinical questions and emergency calls to the medical provider. Documents and closes telephone calls in the EHR in a timely manner per written procedure.   Documents timely and accurate clinical data into NextGen.  Educates clients on good health habits, care gap information, and available services to assist with health needs.  Stocks and cleans the lab, exam rooms and medicine room.   Assures the security of all medications.  Maintains an inventory of stock formulary of medications and reorders as needed.  Administers medications as ordered by the provider after confirming the patient, dosage, and route of administration via established protocols.   Communicates with pharmacies as needed, inquiries about medication lists, calls in prescriptions as directed by the provider.  Assists the providers in clinical procedures and exams as requested.   Performs common office procedures such as dressing changes, EKGs, ear irrigation, and suture removal.  Assists other clinic staff as time allows.  Documents, tracks, orders, and monitors all medications ordered through the Drug Assistance Program (DAP) by authorized providers. Maintains all DAP patient’s applications.   Participates in the Quality Improvement program of Healthcare for the Homeless.  Performs and completes PCMH duties as assigned. Duties include but are not limited to: attending patient huddle, medication reconciliation, timely clinical advice, involved in QI activities, access needs, and care coordination tracking and follow up.  Participates in monthly staff meetings and periodic trainings.  Participates in Electronic Health Record training and other necessary clinic software trainings as necessary.  Participates in program outreach.  Assist HCH staff in case management and recruitment activities for program.  Protects the confidentiality of patients and their medical records.  Provides information to clients regarding community resources and services.  Maintains the cleanliness and safety of the clinic, maintains appropriate documentation, and notifies the Clinical Director of any problems.  Performs CLIA waived tests and venipuncture following specifications in the HCH Policy and Procedure Manual.   Utilizes established standard procedures for positively identifying the patient.  Performs lab quality control procedures and maintains the lab quality control log.  Assist in maintaining inventory of lab and clinic supplies and request reorders as needed to direct supervisor.   Assists with arranging referrals of patients to other agencies and healthcare organizations for services as requested by provider and documents referrals in the patient chart.  Assist maintaining a log of referrals for diagnostic tests (such as x-rays and ultrasounds) and specialist consults; uses the log to determine if reports or consult notes have been received and if not, obtains these for the provider, as needed.  Assists completing prior authorizations of medications and diagnostic procedures.  Follows universal precautions in obtaining and handling samples of blood and bodily fluids and consistently uses personal protective equipment. Maintains a safe, clean, and orderly clinic.  Maintains accurate and complete records in accordance with HCH policy and procedures.  Exercises judgment and sensitivity when investigating the background of clients and determining their needs.   Participates as a team player and has a dedicated interest in achieving HCH goals.  Follows HCH policies and procedures.  Collaborates well with peers and patients, promoting a positive attitude in the clinic.   Assist the Foothills Active Day Center as needed by supporting clinical operations, monitoring participant health needs, and collaborating with the Center Director and staff to ensure quality care and program continuity.  Performs all other duties, as assigned by the supervisor.   QualificationsEducation, Experience & Training:  RN or LPN licensure required; CPR Certification required.    Physical Demands:  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Position requires occasional or frequent moderate physical activity. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands to manipulate, handle or feel; and reach with hands and arms. The employee is frequently required to lift a minimum of 50 pounds and push/pull/exert force up to 150 pounds. Consideration for accommodating lifting restrictions may be provided on a case-by-case basis.    Other Requirements:  Must have a valid driver’s license and the flexibility and willingness to change schedules and duties as needed according to staff availability.  Must be willing to travel the service area as needed. Travel is primarily local or within the service area during the business day, although some out-of-the-area and overnight travel may be expected. TB test is required. Agency policy regarding drug and alcohol testing and criminal background checks applies to this position. Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.
