About the Opportunity
Resources for Human Development (RHD) Â is seeking a compassionate and dedicated Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. The RN plays a critical role in supporting individuals with behavioral health, substance use, intellectual, and developmental disabilities by providing comprehensive nursing services, wellness support, and care coordination.
As a key member of a multidisciplinary treatment team, the RN helps promote health, recovery, and community integration through evidence-based practices and collaborative care planning. The ACT Team operates within a recovery-oriented model, providing comprehensive services designed to help individuals achieve and maintain the highest level of functioning in the community.
Key Responsibilities
- Conduct comprehensive nursing and medical assessments in accordance with clinical best practices, regulatory standards, and program requirements.
- Coordinate and facilitate healthcare services to ensure individuals' medical and wellness needs are addressed effectively.
- Develop, monitor, and support individualized treatment and wellness plans.
- Deliver nursing and health-related services in compliance with agency policies, state and federal regulations, and professional nursing standards.
- Collaborate with individuals served, family members, guardians, healthcare providers, and community partners regarding healthcare needs and treatment planning.
- Monitor medication adherence, health status, and overall wellness outcomes.
- Maintain accurate and timely documentation in electronic health records and other required systems.
- Serve as a liaison between program participants and external healthcare providers and agencies.
- Participate in quality improvement initiatives, data collection, and program evaluation activities.
- Provide after-hours on-call support as required by the program.
- Ensure compliance with confidentiality standards, infection control protocols, patient rights regulations, and all applicable policies.
- Attend professional development opportunities and maintain current knowledge of behavioral health, addiction services, and nursing best practices.
- Support a positive, team-oriented environment while demonstrating initiative and problem-solving skills.
Qualifications
Required
- Associate's or Bachelor's Degree in Nursing.
- Current Delaware Registered Nurse License in good standing or Compact Nursing License.
- Current CPR and First Aid certification.
- Valid driver's license in good standing with a minimum of three years of continuous driving experience and no serious traffic violations.
- Strong communication, organizational, documentation, and record-keeping skills.
- Ability to work effectively with individuals experiencing mental health, substance use, and co-occurring disorders.
- Strong attention to detail and ability to manage multiple priorities.
Preferred
- Experience working in behavioral health, addiction treatment, community-based services, or case management settings.
- Experience utilizing Electronic Health Records (EHR) systems.
- Knowledge of substance use treatment, counseling techniques, and recovery-oriented care.
- Proficiency with Microsoft Office and other business applications.

The Assertive Community Treatment (ACT) Team provides comprehensive, community-based behavioral health services through an interdisciplinary team approach. Team members include counselors, nurses, psychiatrists, substance use specialists, vocational specialists, peer support specialists, and program support staff who work collaboratively to help individuals:

The ACT Team operates within Delaware's public behavioral health system and provides services designed to support individuals 24 hours a day, 7 days a week.
