About the Role
Resources for Human Development (RHD) is seeking a compassionate and dedicated Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. The RN serves as a vital member of a multidisciplinary treatment team, providing nursing expertise and healthcare coordination to individuals with behavioral health challenges, substance use disorders, and intellectual and developmental disabilities.
The ideal candidate will be committed to promoting health, wellness, recovery, and community integration while utilizing evidence-based practices such as Trauma-Informed Care, Motivational Enhancement Therapy, and Medication-Assisted Treatment.
Key Responsibilities
- Conduct comprehensive medical and nursing assessments in accordance with best practices, regulatory requirements, and agency standards.
- Coordinate and facilitate healthcare services to ensure individuals' medical needs are effectively addressed.
- Develop, implement, and monitor individualized health and wellness plans.
- Deliver nursing and wellness services in compliance with agency policies, professional standards, and state and federal regulations.
- Collaborate with treatment team members, healthcare providers, family members, guardians, and community partners to support client care.
- Monitor and support medication management and overall health outcomes.
- Maintain accurate, timely, and complete clinical documentation and records.
- Ensure compliance with confidentiality, patient rights, infection control, and other regulatory requirements.
- Participate in quality improvement initiatives, data collection, and program evaluation activities.
- Act as a liaison between clients and external healthcare providers and agencies.
- Provide after-hours on-call support as required.
- Prepare and submit reports and other required documentation within established timeframes.
- Participate in ongoing professional development and required training activities.
Qualifications
Required
- Associate's Degree or Bachelor's Degree in Nursing.
- Current Delaware Registered Nurse License in good standing or Compact Nursing License.
- Current CPR and First Aid Certification.
- Valid driver's license with a clean driving record and at least three years of continuous driving experience.
- Strong communication, organization, documentation, and record-keeping skills.
- Ability to work effectively with individuals experiencing mental health, substance use, and co-occurring disorders.
- Detail-oriented with strong critical thinking and problem-solving abilities.
Preferred
- Experience working in behavioral health, community mental health, or substance use treatment settings.
- Experience with Electronic Health Records (EHRs).
- Knowledge of addiction treatment, recovery-oriented care, and counseling principles.
- Proficiency with Microsoft Office and other business applications.

The Assertive Community Treatment (ACT) Team provides comprehensive, community-based behavioral health services through an interdisciplinary approach. Team members include counselors, nurses, psychiatrists, substance use specialists, vocational specialists, peer support specialists, and administrative staff working together to help individuals:

The ACT Team delivers a full range of treatment and support services and is funded through Delaware's Division of Substance Abuse and Mental Health (DSAMH).
