Join our team as a Full-Time Home Health Physical Therapist (PT) and provide personalized, one-on-one care where patients are most comfortable — in their own homes.

As a Home Health Physical Therapist, you’ll help patients regain strength, mobility, confidence, and independence while enjoying the autonomy and flexibility that comes with providing care in the home setting.

What You’ll Do

As a Physical Therapist, you will:

  • Perform and document comprehensive initial evaluations to identify functional limitations and establish appropriate treatment goals.
  • Develop and implement individualized treatment plans designed to improve mobility, strength, balance, and overall physical function.
  • Provide therapeutic interventions to restore function, relieve pain, and prevent or minimize physical disability.
  • Evaluate patient progress throughout treatment and adjust plans of care to achieve optimal outcomes.
  • Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and strategies to promote independence.
  • Apply infection control and universal precaution standards during patient care.
  • Collaborate and communicate effectively with physicians, nurses, caregivers, and other members of the interdisciplinary care team.
  • Ensure continuity and quality of patient care through accurate and timely documentation.
  • Complete all required documentation within company guidelines.
  • Follow agency policies, procedures, and professional standards of practice.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current and valid Physical Therapist license in the state of practice.
  • CPR certification.
  • Valid driver's license.
  • Current auto insurance.
  • One year of home health experience preferred.
  • HomeCare HomeBase (HCHB) experience preferred.
  • Strong communication, organization, and time-management skills.
  • Ability to work independently while collaborating effectively with the interdisciplinary team.

Physical Requirements

  • Ability to stand, walk, sit, climb, crawl, squat, and kneel as required to provide patient care.
  • Ability to lift, carry, push, or pull up to 100 pounds as required.
