Providing Coverage at Cedarwood Plaza and Hillside Plaza
Are you passionate about helping residents regain independence, improve daily functioning, and achieve a better quality of life? Cedarwood Plaza and Hillside Plaza are seeking a dedicated Occupational Therapist (OT) to provide therapy services across both skilled nursing and long-term care communities.
The Occupational Therapist will evaluate and treat residents in accordance with physician orders, professional standards, and individualized plans of care. This position works closely with residents, families, nursing staff, physicians, rehabilitation professionals, and other members of the interdisciplinary team.
The selected candidate must be comfortable traveling between and providing scheduled coverage at both facilities.
What You’ll Do
- Evaluate residents’ functional abilities, limitations, needs, and rehabilitation potential
- Develop and implement individualized occupational therapy plans of care
- Provide hands-on treatment to improve residents’ strength, coordination, mobility, self-care skills, and independence
- Assist residents in improving their ability to complete activities of daily living
- Modify treatment plans based on residents’ progress, needs, and physician orders
- Communicate with residents, families, physicians, nursing staff, and other members of the interdisciplinary team
- Participate in care planning, discharge planning, and rehabilitation meetings
- Document evaluations, treatment sessions, progress, and outcomes accurately and promptly
- Train staff and families on techniques, equipment, and interventions that support residents’ therapy goals
- Recommend adaptive equipment and environmental modifications when appropriate
- Promote resident safety and help reduce the risk of falls, injuries, and functional decline
- Ensure services comply with applicable professional standards, facility policies, and state and federal regulations
- Provide occupational therapy coverage at both Cedarwood Plaza and Hillside Plaza based on operational and resident needs
What You Bring
- Bachelor’s degree or higher in Occupational Therapy from an accredited program
- Current and unrestricted Occupational Therapist license in the State of Ohio
- Current registration or certification as required by applicable professional and regulatory standards
- Strong clinical assessment, communication, documentation, and organizational skills
- Ability to work independently and collaboratively within an interdisciplinary team
- Compassionate and resident-centered approach to care
- Reliable transportation and the ability to travel between Cedarwood Plaza and Hillside Plaza
- Experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred
- Experience with Net Health and PointClickCare (PCC) is a plus
