Job DetailsJob Location: ABR - Cleveland, OH 44109Position Type: Full TimeSalary Range: $115,000.00 - $130,000.00 SalaryJOB CONTENT Position Summary: (Overall purpose of this position) Create, maintain, and execute collaborate practice agreements with NFP providers and participates in direct patient care management through a team-based care approach Fulfill the medication needs of patients in a direct care outpatient setting through the processing and accurate dispensing of prescription orders, patient consultation and advisement, and by following state and federal laws and regulations as well as company policy and procedures Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above: (The following description is a general representation of the key duties and responsibilities of this position. Assumes responsibility for entire medication use process; provides direct medication-related patient care, and maintains documentation of such Assumes responsibility for ensuring all healthcare providers have working knowledge of patient drug therapy to ensure quality patient care Evaluate prescriptions are written for a legal and legitimate purpose, process and accurately dispense prescriptions in accordance with state and federal laws and regulations and company policy and procedures Model, implement, and offer guidance and supervision to pharmacy technicians on customer service approaches and practices to ensure all patient needs, concerns, and other issues are addressed in a timely manner in accordance with NFP policies and procedures Medication home delivery process Maintain accurate pharmacy documentation in accordance with state and federal laws and regulations Provide comprehensive patient care by: Applying medical knowledge in the advisement of patients and their medication needs. Informing patients on proper medication practices and maintenance. Educate patients in chronic disease self-management, as applicable. Performing medication reconciliation evaluations on patients. Collaborating with providers on the best treatment plan for patients. Utilizing electronic medical records to assist in the advisement of patient care. Work as a care team member to support NFP as a Patient Centered Medical Home (PCMH). Prescribe or advise prescribing as permitted by license, state pharmacy board, and care agreement. Assist provider and care team in population health activities and monitoring or chronic diseases. Effectively precept students, interns and residents. Maintain and/or perform any necessary preceptor development training QualificationsQualifications: Specific knowledge, training or skills required to perform the duties of this position. Current State of Ohio pharmacy licensure in good standing Bachelor’s degree in Pharmacy or Doctor of Pharmacy degree PGY2 in ambulatory care pharmacy OR PGY1 with focus in ambulatory/community pharmacy OR BPS approved Board Certification in Pharmacotherapy or Ambulatory Care and at least 3 years of ambulatory care practice experience  Current BLS Certification Demonstrated understanding of public health policy and practices; familiarity with medical home model of care In-depth emotional intelligence: demonstrated self-awareness, accurate self-assessment, sensitivity and empathy, openness, reliability and consistency. Demonstrated cultural competency and understanding of underserved populations. Adheres to the NFP attendance policy and will make every effort to consistently be punctual when reporting to work and during daily activities. Demonstrated written and oral communication skills. Demonstrated critical thinking skills and ability to work autonomously. Demonstrated ability to manage multiple priorities and generate results. Demonstrated analytical and problem solving skills. Computer proficiency: application of EMR and related functional software. Previous experience that is necessary background to qualify for this position: Pharmaceutical care giving experience in a community health center, preferred.
