It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. They offer health education, connect people with medical and social resources, and help break down barriers to care, especially for underserved groups. By providing culturally sensitive guidance, assisting with programs like Medicaid and SNAP, and coordinating client services through the Better Health Partnership Pathways Hub Program, CHWs play an essential role in improving community health.   Job Duties: Establish and maintain relationships between vulnerable populations and healthcare or social service systems. Assist clients in navigating healthcare services and public assistance programs. Support care transitions and help reduce social isolation among clients. Provide culturally appropriate health education, including chronic disease prevention, nutrition, and wellness topics. Advocate on behalf of individuals to ensure access to appropriate services and resources. Accurately collect, document, and enter client information into the Care Coordination System (CCS) in a timely manner. Participate in monthly Pathways Hub meetings and collaborate with interdisciplinary care teams. Complete routine administrative duties and maintain accurate, confidential client records. Qualifications Certified Community Health Worker, licensed through the Ohio Board of Nursing. High school diploma or GED required; associate or bachelor’s degree preferred.
