Job DetailsJob Location: Ridge - Cleveland, OH 44102Position Type: Full TimeSalary Range: $60,000.00 - $70,000.00 SalaryJob Purpose: - The RN Coordinator at Neighborhood Family Practice is responsible for overseeing and coordinating patient care activities to ensure the delivery of high-quality healthcare services. This role involves collaborating with healthcare teams, managing patient care plans, and ensuring compliance with healthcare regulations and standards. Key Responsibilities: - Coordinate and oversee patient care activities to ensure efficient and effective service delivery. - Collaborate with healthcare teams to develop, implement, and evaluate patient care plans. - Monitor patient progress and adjust care plans as necessary to meet patient needs. - Ensure compliance with healthcare regulations and standards in all patient care activities. - Serve as a liaison between patients, families, and healthcare providers to facilitate communication and understanding. - Provide guidance and support to nursing staff and other healthcare professionals. - Participate in quality improvement initiatives to enhance patient care outcomes. - Maintain accurate and up-to-date patient records and documentation. - Assist in the development and implementation of policies and procedures related to patient care coordination.QualificationsRequired Education: - Bachelor of Science in Nursing (BSN) from an accredited institution - Current Registered Nurse (RN) license in the state of practice Required Experience: - Minimum of 3 years of clinical nursing experience - Previous experience in a coordinator or supervisory role preferred - Experience working in community health or outpatient settings is an advantage Required Skills and Abilities: - Strong leadership and organizational skills - Excellent communication and interpersonal abilities - Proficiency in electronic health records (EHR) systems - Ability to work collaboratively with multidisciplinary teams - Strong problem-solving and decision-making skills - Commitment to providing high-quality patient care and improving health outcomes - Ability to manage multiple tasks and priorities effectively
