Position Summary
Willow Creek Health Care Center is seeking a compassionate and motivated Full-Time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. The COTA works under the direction and supervision of a licensed Occupational Therapist to provide direct patient care and assist residents in achieving their highest level of functional independence.
This role is essential in helping residents improve activities of daily living, mobility, strength, and overall quality of life within a skilled nursing setting.
Essential Duties and Responsibilities
Provide skilled occupational therapy treatments as directed by the supervising Occupational Therapist.
Carry out therapy interventions in accordance with physician orders and established treatment plans.
Document daily treatment notes, weekly progress notes, and other required records in compliance with state practice acts and payer regulations.
Communicate resident progress, concerns, and treatment updates to the Occupational Therapist and interdisciplinary team.
Assist with discharge planning and caregiver education.
Instruct residents, families, and nursing staff in maintenance programs and safety techniques.
Participate in interdisciplinary care conferences as needed.
Maintain cleanliness and organization of therapy areas and equipment.
Report equipment issues to ensure safe and effective operation.
Maintain professional and respectful communication with residents, families, and team members.
Supervisory Responsibilities
This position does not have supervisory responsibilities.
Qualifications
Education and Experience
Graduate of an accredited Occupational Therapy Assistant program.
Current, active, and unencumbered COTA license in the State of California.
Experience in a skilled nursing or post-acute setting preferred.
Skills and Abilities
Strong knowledge of occupational therapy practices and rehabilitation techniques.
Ability to follow treatment plans and adapt interventions based on resident needs.
Effective communication and interpersonal skills.
Strong organizational and documentation skills.
Ability to work collaboratively in an interdisciplinary team environment.
Proficiency with electronic medical records; PCC experience preferred.
Certificates, Licenses, Registrations
Current Certified Occupational Therapy Assistant (COTA) license in good standing.
Must meet continuing education requirements per state regulations.
Physical Demands
Frequent standing and walking; occasional sitting.
Frequent reaching, bending, lifting, pushing, and pulling.
Ability to assist residents with mobility and therapeutic activities.
Ability to lift and move equipment or assist residents safely.
Work Environment
