Description

The Wraparound Facilitator provides intensive care coordination for identified children, youth, and their families using The NM High-Fidelity Wraparound model. This includes providing care through a team-based and strengths-based approach using the model’s values and core elements. The ultimate purpose of the Wraparound Facilitator is to help families reach their vision through the meeting of their underlying needs, as measured through benchmarks and using predominantly community-based strategies.

Requirements

Full Time, 40 hours, non-exempt

On call required after training is completed

Starting Salary: $41,859.84

Bilingual -Spanish Strongly Encouraged To Apply

A minimum level of education is a high school diploma

Travel required between Clovis and Portales


This position may be home-based at times. The home office needs to meet security measures and the work environment must be free of background noise distractions. Own internet access is a must.

  • Engages by meeting consistently with youth, families, and community stakeholders to assist in identifying visions, goals, and underlying needs in all settings.
  •          This may require non-traditional 8-5 core hours due to school and parent work schedules.   
  • Manages action plans by scheduling, preparing, and hosting team meetings. Disseminates the family’s vision, goals, and needs to the team. Facilitates healthy and proactive discussion in the team setting to strategize underlying needs. After Team meetings, follows up with all team members to check in and follow up on strategy progress and follow through.
  • Manages crisis by engaging the family to develop a Safety and Stability plan to be proactive with stability and safety concerns. Responds as needed to situations that may arise during the program duration. Amends and updates stability plan as needed and as family/youth progress in identifying concerns. Revisits and checks up on safety plan as needed to assist with follow-through.
  • Conducts ongoing outreach to community resources, stakeholders and providers throughout Curry and Roosevelt County communities in order to promote Wraparound and MHR programs.
  • Attend community events in the communities of Curry and Roosevelt counties in order to promote Wraparound and MHR programs.
  • Staffing/ training- 
  •         Foundations Training for one week in Albuquerque in person.  
  •         Meet weekly with Wraparound Coach to meet program criteria and guidelines.  
  •         Work toward Certification by the end of first year as a Wraparound Facilitator in Training and take the certification exam. 
  • Document action plans, electronic health records, BHSD star, and update assessments. Responsible for amending/developing assessments through all phases of wraparound process as well as documenting progress notes, and BHSD services for each service delivered.
  • Maintain HIPPA and client confidentiality at all times.
  • Regular and consistent attendance for the assigned work schedule is essential.
