Willow Creek Healthcare Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the AM shift. Key Responsibilities
Provide direct nursing care in accordance with resident care plans and physician orders
Administer medications and treatments accurately and timely
Monitor residents for changes in condition and promptly report findings
Document care, observations, and changes in condition in the EMR system
Assist with admissions, discharges, and transfers as needed
Collaborate with CNAs and interdisciplinary team members to deliver resident-centered care
Maintain a clean, safe, and supportive environment
Follow all infection control, safety, and confidentiality protocols
Qualifications
Active California LVN license in good standing
Current CPR certification
Skilled nursing or long-term care experience preferred
Familiarity with EMR systems (PointClickCare preferred)
Strong communication, organization, and time-management skills
Compassionate, reliable, and team-oriented
