Job Purpose or Objective(s): The Social Worker processes referrals and completes case assignments. You will work with clients to ensure stabilization and organize activities to enhance social development and meet program goals. You will report to the Program Director or Manager.

 Primary Tasks:

1.      Design program content and organize activities that promote social development of individuals and support program goals.

2.      Guide clients through the application process, selecting programs that align with each client’s developmental levels, needs, abilities, and interests.

3.      Attend team meetings, participate in staff development training, and support continuous quality improvement planning.

4.      Conduct evidence-based assessments of participants and families, ensuring completion by assigned deadlines.

5.      Maintain up-to-date records and communicate updates to the participant to meet compliance standards.

6.      Schedule, conduct, and document visits with participants, including home visits to render services.

7.      Process intakes for tribal members in crisis situations.

8.      Identify resources for tribal members, facilitate connections, provide representation, and document the client’s needs for referrals to community resources or other support organizations.

9.      Complete required federal and program reports, including detailed inventory records for ordering and accountability purposes.

10.  Complete other responsibilities as required.

Required Education and Experience

·         Bachelor’s degree in social science field or four [4] years of directly related experience in lieu of education and:

·         Two [2] years of experience in a social work-related field

·         Victims Academy Certification or National Family Support Professional Certification

·         Ability to mediate at a professional level  

·         Demonstrate a high level of cultural competence, sensitivity, and responsiveness  

·         Knowledge of case planning, case management, and writing skills
