What You'll Do:
- Provide hands-on nursing care in accordance with scope of practice
- Collaborate with RNs and physicians during rounds and care planning
- Coordinate care and communicate changes to residents' families and team members
- Administer medications and treatments, and monitor for effectiveness
- Review and implement physician orders; assist in emergencies, including CPR
- Document all care accurately and timely in the medical record
- Complete admission documentation and assist in discharge planning
- Ensure safe, clean, and well-stocked medication and treatment carts
- Participate in medication management and pharmacy communications
- Follow up on consultant visits and implement recommendations
- Supervise Medication Aides/Techs during medication administration
- Support audits and assist with quality improvement efforts
What You'll Bring:
- Current LPN license (in good standing)

Requirements
Qualifications
Education
Graduate of an accredited RN School of Nursing or LPN Program
Licenses/Certification
Current RN or LPN Licensure in the State employed.
Valid CPR Certification.
Experience
Six months experience in a long-term care environment preferred.
