Full-Time Physical Therapist

As a Full-Time Physical Therapist, you will develop individualized treatment plans, collaborate with interdisciplinary teams, and guide patients through their rehabilitation journey while ensuring compliance with all state and federal regulations.
Patient Care & Rehabilitation
- Evaluate patients' physical therapy needs and develop individualized treatment plans based on physician orders.
- Establish therapy goals tailored to each patient's abilities, needs, and tolerance levels.
- Implement, evaluate, and modify treatment interventions to maximize outcomes.
- Monitor patient progress and adjust care plans to help patients achieve realistic and measurable goals.
- Provide screenings and therapeutic services designed to improve function, mobility, strength, and overall quality of life.
Collaboration & Communication
- Work closely with physicians, nurses, therapy staff, and other healthcare professionals to coordinate patient care.
- Participate in interdisciplinary care planning and contribute to team goals.
- Communicate patient status, progress, and treatment recommendations to patients, families, and care teams.
- Educate patients and family members on therapy techniques, treatment plans, and ongoing care needs.
Documentation & Compliance
- Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
- Ensure therapy services comply with HIPAA requirements and skilled nursing facility guidelines.
- Adhere to Minimum Data Set (MDS) procedures and support effective care planning processes.
Leadership & Support
- Provide clinical guidance, consultation, and supervision to Physical Therapist Assistants (PTAs).
- Promote best practices in therapy delivery and patient care.
- Perform additional duties as assigned to support the rehabilitation and healthcare team.
Knowledge, Skills & Abilities
- Strong understanding of physical therapy principles, techniques, and best practices.
- Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
- Understanding of managed care practices and reimbursement processes.
- Knowledge of state and federal regulations governing therapy services and documentation in skilled nursing facilities.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Passion for patient-centered care and rehabilitation excellence.

Requirements
Education: Â
Bachelor’s, Master’s or Doctorate Degree from a college or university with an accredited Physical Therapy program.Â
Licenses/Certification:
Current Physical Therapist license in the state employed.
Current CPR certification.
Experience:
Six months experience in a long-term care environment preferred.
