The Case Manager Team Lead will direct, plan, organize and leads all activities during a shift of the department staff so that all processes are completed in a timely, efficient manner ensuring quality and the highest level of professional care is provided. Ensures that a high quality of standards are met to service the needs of patients, physicians, regulatory agencies, community agencies, outside entities, and all hospital departments.

Job Requirements

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

  • Graduate of an Accredited School of Nursing
    • Nursing Diploma or Associates Degree in Nursing required, BSN preferred
  • RN License in Applicable State of Employment required
  • Basic Life Support American Heart Association Required BLS certification from the American Heart Association.

  • 4 years as a registered nurse or 4 years as a social worker
  • Excellent communication and presentation skills
  • Ability to multi-task, set priorities, and maintain organizationProficient with computers (including but not limited to Microsoft Office and EHR applications)
