Job DetailsJob Location: Colorado Springs Pikes Peak Location - Colorado Springs, CO 80909Position Type: Full TimeSalary Range: $75,000.00 - $86,250.00 Salary POSITION SUMMARY: The Certified Diabetes Care and Education Specialist (CDCES) works as part of the clinical operations team and is primarily responsible for the clinical expertise related to the care related to diabetes management and education. Working closely with the Chief Medical Officer and Chief Nursing Officer, this position is a key member of the health care team. The CDCES is encouraged to participate in quality improvement, develop relationships with internal and external customers, and help in the implementation and development of diabetes care and education processes and policies. MISSION: Improving lives, Optimizing wellness, Promoting independence COMPETENCIES: Technical Expertise Problem Solving Teamwork Effective Communication Results Oriented Personal Credibility Quality Focus People Focus Flexibility RESPONSIBILITIES AND DUTIES: ESSENTIAL JOB FUNCTIONS: Provides diabetes education to participants with pre-diabetes, Type 1, or Type 2 diabetes. Develops and implements individualized diabetes care and education plans for participants, their families, and caregivers, in collaboration with medical provider. Monitors adherence to plan, and modifies plan as needed. Individualized education plans shall take into account different cultural, physical, cognitive, and literacy levels of participants and families and meet evolving needs of participant Assists medical provider/pharmacist in data collection for continuous glucose monitors. Assists medical provider in evaluating blood glucose logs and recommends modification to treatment plan based on these evaluations Instructs participants, their families, and care givers on preventing, recognizing, and treating hypoglycemia. Assists medical provider in adjusting dietary habits, and individual barriers to care Provides nutritional counseling for diabetes management and weight control. Uses comprehensive knowledge of nutrition to assist participants with meal planning. Teaches participants to manage blood glucose with carbohydrate counting when appropriate Identifies individual social and emotional barriers (such as depression) to compliance with treatment plan, and refer participant to appropriate team member, such as social worker and/or integrated behavioral health counselor Assists medical provider in monitoring participant’s diabetes care flow sheet, ensuring participant gets timely recommended preventive and monitoring tests, treatments, and referrals per diabetes care guidelines Monitors and evaluates short and long-term participant responses to therapeutic interventions in collaboration with medical providers; provides necessary follow-up for participants and their families Performs assessments and data collections, establishes outcome criteria; obtains, reviews, and analyzes information in collaboration with participants, their families, care givers, and health care team members to assess effectiveness of care plans and treatments Evaluates the diabetes care and education program on a continuing basis; assesses teaching methods and education material for staff, participants and their families Assists in the review and/or the development of clinical practice guidelines. Serves as content expert for diabetes education and management Provides updates, educates staff in diabetes; coordinates in-service training, continuing education, and other related diabetes training Develops collaborative relationships with other departments to facilitate and support quality diabetes care. Facilitates coordination of care across specialty care, facility-based care May act as a member of the interdisciplinary team (IDT) as requested Other duties as requested/assigned Qualifications Bachelor of Science in Nursing and/or Dietetics from an accredited facility is required Current certification by the Certification Board for Diabetes Care and Education (CBDCE) as a Certified Diabetes Care and Education Specialist (CDCES) is required Three years of experience caring for individuals with diabetes is preferred Experience working in a multi-disciplinary work environment desired Must be able to demonstrate computer competency including Microsoft Office Applications such as Outlook, Excel, Word, etc. Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
