Overview

We are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. The RN Care Manager plays a key role in assessing, planning, coordinating, and delivering high-quality care to patients in the community setting. This role is ideal for nurses who thrive in a collaborative environment focused on patient advocacy, clinical excellence, and continuity of care.


Primary Responsibilities

  • Perform comprehensive assessments of patients with complex or high-risk needs.

  • Develop and implement individualized care plans tailored to each patient’s goals and medical conditions.

  • Collaborate with physicians, social workers, discharge planners, and other care providers to ensure a coordinated approach to patient care.

  • Facilitate smooth care transitions between settings and providers, ensuring continuity and quality of care.

  • Evaluate and adjust care plans to maintain cost-effective and appropriate levels of care.

  • Identify and address social determinants of health such as housing, transportation, and community support.

  • Promote wellness and illness prevention strategies.

  • Maintain detailed and accurate records of all clinical, functional, and fiscal activities.

  • Serve as a patient advocate while upholding privacy and confidentiality standards.


Qualifications

  • Associate’s or Bachelor’s Degree in Nursing (BSN preferred).

  • Active and unrestricted Colorado RN License or Compact Multi-State License.

  • Prior experience in Hospice Care or Home Health setting preferred.

  • HCHB (HomeCare HomeBase) experience strongly preferred.

  • Reliable transportation and valid auto insurance required.

  • Strong critical-thinking, communication, and decision-making skills.

  • Case Management Certification (CCM or equivalent) preferred.
