Key Job Duties Monitor client health needs, assist with triage, administer medications, and maintain accurate medical records. Schedule and coordinate medical appointments, follow-up care, and transportation to healthcare providers. Collaborate with physicians, pharmacies, families, and other providers to ensure continuity of care and medication management. Maintain clinic documentation and healthcare records in compliance with organizational and regulatory requirements.
