Job Summary

Essential Functions

• Provides psychosocial assessment and interventions. Coordination and collaboration with ancillary services, providers, and agencies.

• Implementation/coordination of identified discharge planning/placement needs. Works closely with others in an effort to optimize quality and minimize cost.

• Provides post-discharge follow-up when deemed appropriate.

• Assists in development/maintenance and evaluation of clinical data. Interprets data and makes recommendations based on results.

• Utilization of paper and computerized charting.

• Supports product line/medical staff quality improvement initiatives.

• Rotational weekend and weeknight calls.

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - Master's degree in Social Work

• Experience - Two (2) years of experience in health-related area

Required Certifications, Registrations, Licenses

• Licensed Master of Social Work (LMSW) or Licensed Independent Social Work (LISW).

• BLS

• Current Heart saver certification

• ACM preferred.
