Mount Carmel is seeking an experienced Home Infusion Pharmacist.The position is Monday through Friday with expectations of on call when needed.
**Sterile Compounding and Home Infusion background highly preferred **
About the job:
Home Infusion Pharmacist provides pharmaceutical care for the outpatient populations according to professional standards and statutory requirements. Directs the functional activities of the home infusion pharmacy technicians, interns, and pharmacy residents. Provides medication dispensing and distribution with a high degree of accuracy and under strict time constraints. Develops safe, rational and cost-effective medication care plans to improve patient outcomes. Manages drug therapy and utilization across the continuum of care and monitors medication therapy outcomes.
What we are looking for:
Education: Bachelor or Doctor of Pharmacy required
License / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio or must obtain Ohio licensure within 90 days of starting.
Experience: Two years recent experience in acute-care setting. Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
Sterile compounding experience is preferred
Effective communication skills
Broad knowledge of medications and dispensing processes. Thorough knowledge of pathophysiology, pharmacokinetics, and therapeutics.
What you will do:
Provide on-call coverage during the week and on weekends to facilitate initiation or continuation of a patients’ home infusion therapy. Document on call activities and if appropriate, communicate with relevant personnel
Participate in teaching programs affiliated with the Department of Pharmacy, and training programs for nursing, physicians, pharmacists, and other allied health professionals.
Communicate effectively with home health nursing, case managers, and other healthcare professionals within and outside of the Mount Carmel Health System via electronic information systems, direct and indirect telephone communications.
Review reported laboratory values from all sources, charts the review in the patient’s pharmacy medical record, and directly communicates all critical values to the physician or designee. Orders labs under the physician’s direction to support the care of the patient.
Participate in patient care conferences in coordination with other patient care providers including, but not limited to, discharge planners, physicians, staff nurses, patients, patient’s families, social workers, home health nursing and other health care providers.
Coordinate all aspects of home infusion pharmacy care between patient’s health insurance provider, delivery agency, billing agency, inpatient pharmacy, contracted admixture compounding pharmacy, and home nursing agency
