Job DetailsJob Location: Columbus, OH 43207-4012Position Type: Full TimeSalary Range: $22.00 HourlyJob Shift: DayThe Service Coordination Assistant supports the coordination of services and the monitoring of service delivery for individuals receiving community-based services. This role ensures that services are implemented as planned, assists individuals and providers in addressing service needs, and promotes continuity, safety, and quality of care. Primary Responsibilities Scheduling, Service Coordination, & Monitoring: Schedule appointments as required or refer and select service providers in accordance with the service plan. Coordinate with service providers to ensure timely and appropriate delivery of approved services. Communicate regularly with individuals and providers to confirm that services are delivered as planned. Monitor service implementation and proactively identify and resolve service gaps. Verify service authorizations and refer billing or authorization issues to the appropriate team. Collaboration & Teamwork: Collaborate with service coordinators and interdisciplinary teams to address overall service needs. Share relevant updates, concerns, or changes in the individual’s condition or environment with appropriate team members. Individual and Provider Support: Educate individuals on service options, including rights to appeal and self-directed services. Assist with transportation coordination and other supportive services. Request additional assessments or evaluations (e.g., therapy) when needed. Notify service providers of changes in eligibility or participation status. Documentation & Reporting: Document all correspondence with providers and program members in accordance with program guidelines. Adhere to established documentation practices and standards. Generate Monthly Activity Reports (MARs) on resource utilization and the status of member referrals. Maintain discretion and adhere to confidentiality protocols. Educational Support: Provide members and families with information on available resources and how to access them. Support onboarding by meeting with new staff to explain the Coordination Assistant role. Participate in team and staff meetings to share insights and updates. Complete all required trainings and meetings within program timelines. Consult with professionals to build knowledge and skills relevant to the role. Safety & Compliance: Report concerns related to provider performance or service issues to the appropriate team. QualificationsMinimum Requirements: High school diploma or equivalent. 1 year of experience in social services, case management support, or healthcare coordination. Skills Strong communication and organizational skills. Ability to manage multiple tasks and maintain accurate records. Comfortable working independently and as part of a team. Basic computer skills and familiarity with electronic documentation systems. Working Conditions Traditional office environment with standard business hours.
