Position Details:
- Main Office location: Concord, MA
- Service Area: Concord Center Territory
- Job Type: Full Time
- Schedule: Monday - Friday
- Salary Range: $100,000.00 - $115,000.00
Responsibilities:
- Lead and support the Center's clinical team, providing day-to-day guidance, coaching, and problem-solving to help clinicians deliver exceptional patient care.
- Oversee daily clinical operations, including new admissions, patient assignments, visit completion, staffing needs, and continuity of care.
- Lead daily clinical meetings to keep the team connected, address patient needs, identify barriers, and coordinate priorities. (In person once per week and virtual the other four days)
- Serve as a clinical resource for staff, assisting with complex patient situations, care planning, documentation, physician communication, and other clinical needs.
- Monitor patient care and clinical documentation to promote quality, regulatory compliance, timely billing, and positive patient outcomes.
- Manage and develop team members through regular feedback, joint visits, case reviews, competency assessments, performance evaluations, recognition, and ongoing education.
- Partner with Recruiting and leadership to identify staffing needs and participate in interviewing, selecting, onboarding, and retaining clinical team members.
- Monitor clinician schedules, productivity, and caseloads while balancing patient needs, staff capacity, and appropriate use of per diem resources.
- Support quality and safety initiatives, including incident review, infection control, patient feedback, staff education, and performance improvement efforts.
- Collaborate with Administrative, Marketing, and other clinical leaders to support referrals, census growth, efficient operations, and a positive patient and employee experience.
- Maintain clinical competency and provide patient or joint visits when needed to support patients and team members.
Qualifications:
- Active license as an RN, PT or OT in MA.
- Maintain current BLS CPR certification.
- Minimum of two (2) years of direct clinical experience; home health preferred.
- Prior supervisory experience preferred.
- Excellent written and verbal communication skills.
- Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.
