A Day in the Life as a Long Term Care Options Counselor :

As a Long Term Care Options Counselor you’ll provide person-centered options counseling for people seeking long term support and services for themselves or loved ones in a variety of settings including the persons home or other community spaces. Your day will involve:

  • Performing person centered, comprehensive needs assessments, including mental, physical, functional, cultural, financial, environmental, and life goals, to determine appropriate referrals.
  • Developing preliminary care planning and long-term care counseling. Conducts information gathering, and provides limited follow-up based on consumer preferences.
  • Providing long term care counseling to select persons in hospitals, rehabilitation facilities, nursing homes, or in-home settings.
  • Assisting with providing periodic public education sessions

Your role will directly contribute to a positive participant experience through the ADRC person centered model.

Requirements

Qualifications to Be a Long Term Care Options Counselor :

To excel in this role, you’ll need:

  • Education: Bachelor’s degree in Human Service related field preferred or Associate’s degree with two additional years of relevant experience or other related certifications (CHW License, SHIP certification, etc…) with three years of additional relevant experience
  • Experience: Experience working with older adults, adults with disabilities, and their caregivers required. Experience working with basic health insurance principles preferred.
  • Skills: Ability to communicate effectively, summarizing data, prepare reports, and make recommendations based on findings to achieve work objectives.
  • Characteristics: Good interpersonal skills, openness and flexibility in working with diverse groups, and enthusiasm for working collaboratively and with a team.
  • Technical: Knowledge of the principles and methods of person centered counseling for older adults, adults with disabilities, and their caregivers, and their relationship with the people they care for, focusing on options counseling using public, private, and natural resources.

Reliable transportation and a valid driver’s license are required, as some travel within the community may be necessary.
