We’re looking for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to work with our clients in or near Concord, New Hampshire. You MUST have an active license in New Hampshire to be considered for this role. RNs or LPNs are also welcome to apply.Â

As a home care nurse, you will provide care to patients in both a home setting and independent living facilities. Your daily responsibilities will include collaborating with the caregiving team, patients, and families to provide the highest level of care.Â

Job Details:Â
- Competitive pay, up to $45/hour availableÂ
- Participate in administrative responsibilities, including documentation and communication with the office team, with paid admin time providedÂ
- Flexible full-time and part-time hours available- build your own schedule. Â
- Weekends not requiredÂ
- Evaluate clients and develop care plans for clinical and non-clinical teams to followÂ
- Provide training and education to patients, families, and care teams to ensure safety in the home and promote independence.Â
- Work alongside the care team to develop care plans and complete documentationÂ
- Assist with promoting client independence and healingÂ
- Demonstrate competency in carrying out patient care/treatmentsÂ
Requirements:Â
- Must have an active RN or LPN license in the state of New HampshireÂ
- 1 to 2 years prior experience Â
- Active CPR certificationÂ
- Valid driver’s license and reliable transportationÂ
- Clean driving record and background checkÂ
- Per State regulations COVID with BOOSTER and flu vaccinations mandatory unless medical exemption presented. Â
