Position Details:

Hartford Hospital is seeking a Physician Assistant or Nurse Practitioner to join their Department of Medicine team! We are looking for a non-clinical APP to conduct outreach post discharge calls to complex patients, ensuring discharge instructions are adhered to. Go over medication reconciliation lists and confirm prescriptions are filled and picked up. Establish follow up with PCP/specialists.

Role: Open to Physician Assistants or Acute Care Nurse Practitioners

Location: 80 Seymour Street, Hartford, 06102

Setting: Inpatient

Schedule: Per diem (20-24 hours weekly)

Call schedule: No call

Nurse Practitioner and Physician Assistants Role Scope:

      Utilize Epic AI model to identify high-risk patients at discharge

      Reduce readmission rates for high risk, high VVI score patients

      Target patients with ≥22%25 predicted risk of readmission

      Focus on those discharged home, not to a facility

  • Licensure: Unrestricted State of Connecticut Medical License (or eligible)
  • Training: Certified or board-eligible Physician Assistant or Acute Care Nurse Practitioner
